Examiners noted there was no date on opened goat curd and yoghurt.
They also found “mould forming on cheese”.
Meals cost up to £100-a-head at the restaurant in Axminster, Devon.
But it was given the second lowest rating for hygiene.
Inspectors found there was a danger of an E.coli outbreak — which can be fatal — because the kitchen was putting raw food in the cooked food vacuum-packing machine.
The report carried out by East Devon district council said the “young chefs” had “little knowledge or understanding of the high risk imposed by E.coli”.
Boss Hugh, 60, filmed Channel 4 series River Cottage from 1999-2013, including a three-part series at the Devon HQ in 2012.
According to his website, River Cottage “offers a warm, dog-friendly space to enjoy breakfast, lunch, drinks and cakes with stunning views of the Axe Valley”.
A spokesman said: “Having rectified everything raised in the initial report the inspector will return to formally assess our improvement within days and we hope to reinstate our five-star rating.”
