LUCY Letby investigators will examine management across the wider NHS in the public inquiry into the jailed nurse’s crimes.
Ministers yesterday laid out the scope of the inquiry, which will be led by High Court judge Lady Justice Thirlwall.

It said it will cover “the effectiveness of NHS management and governance structure in keeping babies in hospital safe” and “whether changes are necessary”.
The section will include “a consideration of NHS culture” and look at “how accountability of senior managers should be strengthened”.
The Letby case led to calls for tougher regulation of NHS managers after it emerged that bosses failed to act when consultants raised concerns that she was harming tots.
Many now want to see bosses on a professional register like the General Medical Council, which is mandatory for doctors.
The wider NHS scope will be in addition to specific investigations of what happened at the Countess of Chester Hospital where Letby worked.
Health Secretary Steve Barclay said: “I cannot begin to imagine the hurt and suffering experienced by the families affected by Lucy Letby’s horrific crimes.
“We have a duty to get them the answers they deserve, to hold people to account where they need to be, and to make sure lessons are learned.”
