The Argentine, 20, has been less prominent in United’s team since Ruben Amorim‘s arrival.
He was dropped from the matchday squad altogether for the Manchester derby victory over City last month.
And he has since had to be content with coming off the bench.
According to Sport Italia, Napoli boss Antonio Conte is determined to sign Garnacho.
The Serie A side are set to lose star winger Khvicha Kvaratskhelia.
Conte, 55, this week revealed: “Kvara has asked to be sold. The club told me and he confirmed it.
“I feel very disappointed, because in these six months I have put him at the centre of the project making him understand that something important could be done and I have worked with the club for the renewal, instead we are back to the beginning.”
Kvaratskhelia is said to have agreed personal terms with Paris Saint-Germain, as the French side negotiate with Napoli over a fee.

With the Georgian set to depart, Garnacho could be recruited as his replacement.
Sport Italia’s report claims that the United star may be available for just £42million – with fans suggesting that figure “must be a typo”.
Garnacho has played 115 times for the Red Devils, scoring 23 goals and assisting a further 13.
This season he has three goals in 19 Prem outings, with just one assist.
Due to Garnacho being a homegrown player, a transfer fee received for him would count as 100 per cent profit with regard to PSR rules.
The Red Devils are eager to get ahead of potential PSR issues as they aim to reshape Amorim’s squad.
Academy starlet Kobbie Mainoo has been the subject of shock exit reports, with Chelsea reportedly keen.
While Marcus Rashford has also been linked with AC Milan and Borussia Dortmund this month.
United are set to complete their first signing under Amorim.
