Tamworth, who are 16th in the fifth tier National League, caused Spurs problems throughout.
Winger and Zara shop assistant Beck-Ray Enoru was a threat, sandwich maker Tom Tonks’s long throw panicked them and bricklayer Dan Creaney lead the line with physicality.
“Premier League you are having a laugh” and “you are getting sacked in the morning” chanted the Tamworth fans at Spurs and Ange Postecoglou.
Both teams had chances to win it in normal time.
But in extra-time, Tottenham got the goal as Nathan Tshikuna failed to sort his feet out and turned a low cross into the box.
Dejan Kulusevski and Brennan Johnson then doubled their lead.
